Is there a watch to monitor heart rate that can send alerts for my mom to my phone?

I’m searching for an apple watch app that can send alerts to an emergency contact when there is a large change in HR or a fall detected. My elderly mother lives alone in disabled living apartments and I can’t go see her at this time due to working in healthcare and not wanting to expose her. If anyone has recommendations for a Apple Watch I can buy/ program to do these things I would greatly appreciate it! She has an iPhone if that makes a difference in the programming of the watch I would have to do.
